Who We Support
Our Community Outreach service supports adults with learning disabilities across North Yorkshire. We work with individuals who need a little extra help to stay confident, connected and independent, whether they live alone, with family or in supported accommodation.
What Support Is Available?
We offer friendly, practical support with everyday life that is tailored to each person’s needs. This might include:

Help with shopping, banking or bill payments

Getting to and from appointments

Encouragement to attend groups or activities

Building confidence and daily routines

Light support at home or out in the community

A regular check-in or friendly chat
Everything we do is person-led, built on trust and shaped by the individual and the people around them.
Who Delivers the Support (and Safeguarding)
Outreach is delivered by a small, trusted team of Chopsticks support workers. You'll see the same familiar faces regularly, helping build consistency and confidence.
All staff are:
DBS-checked
Trained care personnel
Friendly, reliable and respectful
We’re proud to deliver support that’s safe, flexible and valued by the people we work with.
How It’s Funded
Chopsticks is an approved non-regulated provider, commissioned by North Yorkshire Council.
Some people are referred and funded through adult social care, or through NHS if there is a EHCP in place, others choose to pay privately via direct payments. We’re happy to talk through the options and guide you through the process.

We know that small steps can make a real difference. Whether it’s helping someone return to a favourite café or simply making life feel a little easier at home, we walk alongside each person in the way that works best for them.
